Contact Form with new Business Domain

I changed my business to an LLC in July and made all the changes on Honeybook.  I changed the domain in my company settings to reflect the new business name.  I noticed that my contact form’s domain still has the previous business name in the link that is sent to potential clients.  I set-up a new contact form today, thinking that would fix it, but the old business name is still being used. It’s not a huge deal, but all the forms should reflect the new name, and I can’t figure out how to fix it.  Am I missing something?
