How do you create a contract or smart file template with checkboxes?

I can’t figure out how to create a smart file or template or contract or whatever that includes mandatory checkboxes.  For context, I am attempting to create a release of information that has the client entering some information (therapist name, for example) and requires them to choose what kind of information that can be shared.  I also want them have to initial the document in two places and then sign at the bottom.  Is this possible within HoneyBook?

    Hi Sue -- I think you can create a contract with mandatory checkboxes, BUT you’ll also require signatures (it’s a non-deletable field).

    I figured out this workaround by doing Add Block > Questions > Text approval (for the mandatory checkboxes). You can then Add Block > Contract above it and you’ll have your contract (with the mandatory signatures) there.
