Flowing all data from form into Honeybook via Zapier

Has anyone successfully captured data from a non Honeybook form, then had Honeybook create a project with that information using Zapier triggers?

My zap is sort of working. It will create a new project, and it will flow the name and email address over, but it's not flowing the details over. And it's using my contact form fields from my website instead of the fields that are listed in the Zapier setup.

      Is the Zap giving the same info as the chrome extension would?
